Eligibility Requirements:
- Only permanent employees who are actively working a minimum of 24 hours per week
- Applicant's business must be established and active for a minimum of one year
- All benefits are subject to medical underwriting and all employees must complete a Medical Questionnaire
- Some occupations are not eligible for LTD coverage due to nature of risk
Mandatory Benefits:
- Life insurance $25,000
- AD&D $50,000
- Dependents Life Spouse $5,000 Child $2,500 (only applicable if applying for family coverage)
- Extended Health Care Benefits
- 80% Co-insurance, Survivor Benefit 6 months
- Prescription Drug Plan, $2,000/yr./person, drug card provided
- Practitioners, $400/yr./practitioner (Chiropractor- $20/visit)
- Eye Exams, 1exam/24months ($50 maximum)
- Out of Province/Canada, 60 days/100% co-insurance, $2M maximum, Emergency only

Optional Benefits:
Basic & Preventative Dental Care
- 80% co-insurance, based on the current year's Dental Fee Guide
- Maximum $1,000 per calendar year, per person.
- Recall not more than once every 9 months
- Includes routine oral examinations, bi-tewing x-rays,scaling,polishing, floride applications,fillings, space maintainers, extractions, denture repairs, endodontics and periodontics
Long Term Disability
- 60% of monthly insured earnings to maximum of $2,000 per month, to earliest of recovery, retirement, or expiry at age 65
- LTD payable after satisfying a 119 day (17 week) elimination period
- One year own occupation definition
- If non-taxable LTD benefit is desired, all employees must pay 100% of LTD premiums and applicable taxes
